Due to the national emergency surrounding Coronavirus (COVID-19) outbreak the Orofacial Pain and Oral Medicine Center is closed to the public, however we continue to serve our local community in Los Angeles and beyond in California, through online consultations.

Items to bring to your appointment:

• Insurance card(s)
• Government issued ID
• Your current medications list
• X-rays, CT scan or MRI report related to your visit
• Most recent lab test (CBC/CMP) if available
• Your sleep report if you are coming to get sleep appliance

Optionally if you wish to save time you can download and fill the clinical admission forms. Clinical admission forms are available in English and Spanish.
